  • PLEASE NOTE: If you are surrendering more than one dog to Peggy Adams, you will need to fill out a form for each pet. However, you will only need to schedule one appointment to surrender all of your pets. Once you schedule an appointment for the first pet, the second pet will automatically be added to the original time. Do not schedule a new appointment for each pet.

  • Circumstance of bite: 

    The difference between a provoked and an unprovoked animal bite lies in the circumstances leading up to the bite:

    Provoked Bite:

    • A bite is considered "provoked" when the animal is responding to a specific action or behavior that it perceives as a threat or irritation.
    • Examples include trying to take food away from the animal, startling it while it's sleeping, cornering it, stepping on it, or any other action that the animal might interpret as aggressive or threatening.
    • In these cases, the bite is a defensive reaction.

    Unprovoked Bite:

    • An "unprovoked" bite occurs when an animal bites without any apparent reason or external stimulus that would trigger a defensive reaction.
    • This type of bite might happen when the animal is not being interacted with in any obvious way, such as when it's simply approached without any aggressive action, or when the bite happens without warning or clear cause.
    • Unprovoked bites can sometimes be more concerning as they might indicate underlying issues like illness, neurological conditions, or behavioral problems.

  • We at Peggy Adams Animal Rescue League (Peggy Adams) understand that it is sometimes necessary to relinquish a pet. Since we are committed to keeping animals as long as it takes to find them a loving home, we can only accept as many animals as we can humanely house and care for. 

    Our goal is to find a happy home for every healthy and treatable animal. While we do our best to place every animal that comes in to our care, we cannot guarantee placement for any animal. While it is always a last resort, we will euthanize an animal if deemed medically or behaviorally necessary.

    By submitting this form, you acknowledge the following:

    • The information provided within this form is accurate to the best of my knowledge. I have not misrepresented this animal to Peggy Adams in any way, and understand that any incorrect information about this animal may effect its final outcome once taken in to the League's care.
    • You understand that completing this form and scheduling an appointment does not guarantee that this pet will be accepted into Peggy Adams. Peggy Adam's Admissions team may reach out before the scheduled appointment to gather more information, and may determine at that time this pet cannot be accepted by Peggy Adams, leading to the cancellation of the appointment.
    • You understand this appointment is dependent on the shelter's capacity for care, so this appointment may be postponed if Peggy Adams is over capacity. You understand that during the appointment, this pet may be examined by a medical and/or behavioral professional who will make the final determination on its admission into Peggy Adams.
    • You understand that this pet's admission into Peggy Adams does not guarantee that it will be placed up for adoption. You understand that if you are surrendering your own pet, a non-refundable $50 Care Fee will be collected at the time of this pet's admission to Peggy Adams. This fee will go towards the care of this pet during its stay at Peggy Adams. You understand that payment of this fee does not guarantee that this pet will be considered a candidate for adoption.
    • If this pet is unable to be accepted into Peggy Adams, you understand it will be your responsibility to re-home or otherwise determine the final outcome for this pet.
