Volunteer Liability and Confidentiality Release
I, Peggy Adams Animal Rescue League volunteer (and/or offsite event volunteer), hereby release the Peggy Adams Animal Rescue League from any and all responsibility and liability of any nature whatsoever in connection with any and all activities in which I may participate on behalf of or in any way associated with the League, and any and all injuries of any nature whatsoever which may arise out of, result from, or be associated with said activities. I understand that when working directly with animals there is an inherent risk of bites, scratches and exposure to zoonotic diseases.
I understand that all records, information, cruelty cases, and ongoing investigations are to be kept strictly confidential. I further understand that records, information pertaining to clients, donors, staff, and other volunteers is confidential in nature. Furthermore, I will comply with all regulations, policies, and procedures of the League, and understand that my failure to do so will result in my termination as a volunteer.
I, intending to legally bind myself, administrators, personal representatives or assignees or any party holding a durable power of attorney for my actions, do hereby release the Peggy Adams Animal Rescue League, its employees, Board of Directors, volunteers, or any other individuals associated with the Peggy Adams Animal Rescue League from responsibility for any injury or damages to either myself or my pet as part of my participation in any offsite event or any related activities. I grant permission to any and all of the foregoing to use any photograph, motion pictures, videotapes, or any other record from the event for any lawful purpose.
If the participant is under 18 years of age at the time of registration, the participant's parent or legal guardian must completely review and sign an additional minor waiver. The parent or legal guardian understands and consents to its terms and authorizes the participation of the registrant by his/her acceptance below.
As part of the application process, I UNDERSTAND the League will do a background investigation through a third-party provider of such services. By submitting this application, I consent to the background investigation and to providing information reasonably requested by the League or the third-party provider to complete such background investigation. No Applicant is guaranteed to be accepted as a Volunteer. The League may refuse any Applicant for any reason, or for no reason at all.
I understand that moving forward in the volunteer orientation process, I am REQUIRED to make a weekly commitment to the League for the duration of at least a three-month timeframe. I understand that it is my responsibility to communicate with my Volunteer Lead and the Volunteer Manager when I am planning to be absent from a scheduled shift.
I hereby grant PAARL and its representatives the right to take, use, and publish photographs, videos, or other media of me taken during volunteer activities for promotional, educational, or documentation purposes. I understand these images may appear in print, online, or in other media formats. I release PAARL from any claims for compensation, damages, or ownership related to these images.
I hereby execute this release freely and voluntarily knowing the same is a legal document, and recognizing that I had the opportunity to seek legal counsel pertaining to this document.
